It does happen from time to time. There was a notorious incident a few years back when two young engineers (aged 19 and 21) were on a turbine that caught fire, with no means of escape. Both sadly died. |  |

Yes, they were carrying out a major component exchange hence why the walking barge is on site. There's 1001 reasons why it would've caught fire but most likely human error is the root cause.
